Chemical Composition

Meldonium, also known as Mildronate, is a synthetic analog of gamma-butyrobetaine. It is a white crystalline powder, soluble in water and various organic solvents.

Mechanism of Action

Pharmacodynamics: Meldonium acts as a cytoprotective and cardioprotective agent. It inhibits gamma-butyrobetaine hydroxylase and reduces the synthesis of carnitine, thereby decreasing the transport of long-chain fatty acids across mitochondrial membranes. This leads to a shift in cellular energy metabolism from fatty acid oxidation to glycolysis.

Pharmacokinetics: Meldonium is rapidly absorbed after oral administration. Peak plasma concentrations occur within 1-2 hours. It is predominantly excreted unchanged in urine. The half-life is approximately 4-6 hours.

Indications for Use

Meldonium is indicated for:

  • Treatment of cardiac disorders such as angina, myocardial infarction, and chronic heart failure.
  • Reduction of physical and mental stress.
  • Treatment of cerebrovascular disorders.

Contraindications
